I/we, the parent(s) legal guardian(s),
give permission for (include all children's names):
to participate in the Grace Soccer League, Airdrie, AB, every Thursday, from April 2024 to June 2024.
I/we the parent(s)/legal guardian(s) DO agree to release from liability and indemnify Grace Baptist Church and Grace Soccer League and any of its leaders/designates for any accident or incident that should arise during the timeframe of April 2024 to June 2024 while they are participating in the Grace Soccer League.
We have ensured that the above-named person is in possession of their Health Care number so that they may produce it in the event of need and emergency contact numbers in the same case.
Grace Soccer reserves the right to cancel or postpone soccer due to inclement weather or the threat of inclement weather. All soccer related updates will be communicated through the Grace Soccer League Facebook page.
Each Grace Soccer participant is required to have a parent/guardian present on or around the field of play during practice/game play.
We will do our utmost to ensure none of the above should ever need to come into effect. We look forward to having your child join us this season! The Grace Soccer Team
