Airdrie Grace Baptist Church

2021-2022 GRACE YOUTH ACTIVITIES PERMISSION FORM

This form is for you to give your permission and consent for your child(ren) to participate in the various activities, events, and trips that are a part of the Grace Baptist Church Airdrie Grace Youth. These events include, but are not limited to: retreats, beach runs, service projects, concerts, sporting events, shopping trips, and more.

This information will be kept on file, in a locked filing cabinet, and you will not be asked to fill out another form, unless a third party requires it. This form will be valid from July 2021 – August 2022.

Note re: Medication: If it will be necessary for your child to take prescription medication during a trip, the parent/guardian must inform the Ministry leader in writing before the child will be allowed permission to go on the field trip/excursion.

Advil and Tylenol: Completion of this form grants ministry leaders to provide and offer legal and reasonable use of Tylenol/Advil. (That is, unless you have specified an allergy to Ibuprofen/Acetaminophen). If you DO NOT wish your child to receive Advil/Tylenol, please indicate so in the form under “other medical or special concerns/information.”

Note re: Photos: Photos are a great way for us to share with our church family all the wonderful things that are happening in our ministries! Completion of this form grants Grace Airdrie acceptable usage of ministry related photos. If you do not wish for your child to be in any of our promotional material (social media, emails, website, etc.), Please contact the specified ministry leader. Please note that if you do not wish to have your child in a photo, we may opt to blur the individual's face from said photo. 

Note re: Element of Risk: The risk of injury exists in every field trip/excursion and regular youth activities. However, due to the nature of some activities, the risk of injury may increase. Injuries may range from minor sprains and strains to more serious injuries. There is also a risk to exposure of Covi-19/other pathogens when your child attends any youth activities. The safety and well-being of your children is a prime concern, and attempts are made to manage as effectively as possible the foreseeable risks inherent through compliance with government issued regulations.

My child(ren) has my permission to participate in the various activities, events, and trips that are part of the Grace Baptist Church Youth Ministry, for the period of July 2021- August 2022.
